Intensive Residential Treatment Services (IRTS) facility primarily for low-income older adult. Provides housing, and personal/supportive care. Services include nutritional meals, medical/dental, psychiatric/mental health therapy, social, personal/supportive care, and occupational/physical therapy.

Personal Assistant (PA): Provides assistance with household tasks, personal care and, with permission of a doctor, certain health care procedures. Personal Assistant (PA) are selected, employed and supervised by the individual customer.

In the Individualized Supportive Living program, services and training are provided in the individual’s home and in the community, allowing functional skills to develop in real-life settings. The program’s flexibility allows for individuals with even the most severe developmental disabilities to learn new skills and become active members of the community.

Supported community living programs are provided for persons with mental illnesses who do not have a place to live or who need more structured services while in the community. These programs range from nursing homes to apartments and other standard living accommodations in the community. Persons in these programs receive support through case management and community psychiatric rehabilitation programs provided by administrative agents. Support may include housing, health and safety, budgeting, personal care, home maintenance, nutrition and meal planning and more.

• Shared Living: Individuals with developmental disabilities chose a companion to live with to provide an opportunity to build social skills, independence and companionship. Companions provide 24/7 guidance, ensure individual eats healthy meals and takes medication. Companions are carefully screened, trained and evaluated regularly.

Community Integrated Living Arrangement (CILA): Family style home where residents are evaluated and receive speech, occupational and physical therapy as needed. Recreational programs are also available to residents to develop social skills and improve quality of life.

Provides day and residential support services for the developmentally disabled.

Agency-directed in-home attendant program for individuals with physical disabilities or health problems; services include housekeeping, cooking, homemaker chores, diabetic nail care, respite care, approved nurse visits, diabetic nail care, etc.; agency provides attendant or nurse; also offers Healthy Children and Youth Attendant Care and Veterans Attendant Care.

JCDS offers a variety of residential services and supports to adults with intellectual and  developmental disabilities that are designed around the specific needs of each person. Services range from 24-hour supports to drop-in services.

Residents gain confidence by meeting goals and becoming more self-sufficient. The program gives those who need protection and supervision the opportunity to be a part of the community.

Independent Supported Living Arrangement: Offers support and skills training in the following areas locating and securing housing, health and safety, money management, budgeting, and banking, personal hygiene, home maintenance, nutrition, meal planning, shopping, and cooking, transportation, social skills and friendships and developing natural supports.

Consumer Directed Services: Provides personal care attendant services enabling individuals with disabilities to live independently. Participants in the program are trained to hire attendants to assist them with tasks of daily living.

Individuals are supported in their own apartment according to individual need and may range from a few hours of support per week to 24 hours per day.

Independent Supported Living: Provides 24 hour staff support to people living with disabilities. Staff members assist program participants with personal care, cooking, medication, arranging transportation, coordinating medical care, community outings and social activities.Independent Supported Living Assistance: Provides up to 10 hours of staff support per week to people with disabilities living in their own homes. Participants learn new skills in primary areas of medical, shopping, laundry, housework, safety, finance, transportation, jobs, socialization and cooking.

Provides supports and services to individuals with disabilities who are unable to access waivered services or on waitlists for Medicaid. Support is personalized, teacher-based, focusing on developing long-term independence.

Residential Services: Provides 24 hours a day support to adults with intellectual and developmental disabilities who reside in group or community-based homes. Independent Supported Living: Participants receive needs-based, one-on-one supports that are designed to maximize a person's independence, while providing up to 12 hours of assistance each week.
